This questionnaire is not meant to take the place of getting to know you but will provide a basis to begin our discussion to determine if a Good Life Puppy is a good match for your family. Once you have completed this introduction, we will contact you to continue the get-to-know-you process.

Please note: We do not ship puppies, so you must be willing to come to Michigan to pick up your puppy.

We are happy to provide you with references to current Good Life families upon request. Also, we encourage our potential puppy owners to ask questions of us. This interview process goes both ways! As much as we want our puppies to go to the very best homes, we also want our puppy buyers to be comfortable with us. We hope to be a friend and resource for the duration of your puppy’s life.
